What Is An Adrenal Gland Surgery?

Removing the adrenal gland from the body with a laparoscopic surgery is termed as Adrenalectomy. The two triangular shaped adrenal glands have the function to produce hormones like cortisol, adrenaline, and sex hormones. The organ plays an important role in controlling the number of chemicals, blood pressure, and the use of glucose and water. These endocrine glands are situated in the upper part of our kidneys. The adrenal glands produce aldosterone hormones.

When Is The Adrenal Surgery Done?

The adrenal gland surgery is done when the organ develops some disease or malfunctioning although it is quite rare. The conditions for which the gland is removed include tumor growth inside the organ that leads to a very high hormone production. Usually, non-cancerous benign tumors are found that are small. It is easy to remove such growths by doing the laparoscopic surgery. If the tumor is big or a cancerous and malignant tumor has developed, it may need the removal of the adrenal gland to treat the condition.  Cancerous tumors are very rare. Some patients find that they have a tumor in the adrenal gland when an X-ray is done for some other disease.

Tests For Adrenal Surgery

If the patient has any symptoms of the adrenal tumor, the doctor performs some tests to detect the disease and confirm it. It includes tests like blood tests and urine tests that help in finding the excess production of hormones by the gland and the presence of a tumor. The doctor can do the CT scan and MRI to find the exact location where the tumor has developed. Selective venous sampling is also done.

Open Surgery And Laparoscopic Surgery

While in the earlier times, the adrenal gland removal was done by the traditional open surgery that involved making a big incision, the patients have the option of doing the adrenal gland removal with a laparoscopic surgery today. There is minimum pain and discomfort as the surgery is minimally invasive.

Open Surgery And Laparoscopic Surgery

Preparations Before The Surgery

Medicines are started for people who have pheochromocytoma a few days before the surgery for controlling the blood pressure. If the tumor produces aldosterone hormone, the potassium levels are checked. Potassium is given to the patient in case the potassium levels are found to be low. Cortisone medicine is given to people who have Cushing’s syndrome before the surgery and the medicine is continued post surgery too. EKG, blood tests, and chest X-ray is done before the surgery to check the medical condition. The surgery may need blood transfusion and arrangement is made for the same. The patient is asked to take a shower on the previous day night and or on the day of the surgery.

Precautions On The Day Of Surgery

Foods and drinks are stopped on the previous day night. Medicines for blood thinning and inflammation are stopped before the surgery. Also, Vitamin E is stopped. The patient needs to quit smoking.

Process Of The Adrenal Surgery

The surgeon gives general anesthesia for doing the surgery. Very small incisions of a size one-fourth to half an inch are made to do the surgery. The surgeon makes 3-4 incisions for this. A laparoscopy with a camera is inserted into the body. The camera allows the surgeon to see the internal area of the body clearly. Cannulas are used to cut and remove the adrenal gland, which is taken out from the incisions. This helps in removing the tumor completely. The incisions are closed after the surgery.

When Is The Open Surgery Done?

The laparoscopic surgery is not suitable for some patients. It includes obese people and those who have scar tissue due to an earlier stomach surgery. The surgeon can choose to do the open surgery when there is bleeding during the surgery and if the tumor is big. Open surgery is a good option when the tumor has penetrated into surrounding structures.

Post-Surgery Care And Recovery

The patient has to be hospitalized for 1-2 days for the adrenal gland surgery. People who have pheochromocytoma need to be admitted to the ICU post surgery. After the discharge from the hospital, the patient is advised to do light activities when they return to their homes. There is a slight pain after the surgery and pain medicines are given to control it. The patients can start doing normal activities and return to their work in seven days time. Consult the surgeon again after two weeks of the surgery for a follow-up.

Benefits Of The Laparoscopic Surgery

The laparoscopic surgery has a faster recovery time and a person can return to normal life and start going to the office in a very short time. On the other hand, the open surgery has a longer recovery time and it takes a longer time to return to normal life than a laparoscopic surgery.
